Thermodynamics of weight loss diets

It is commonly held that "a calorie is a calorie", i.e. that diets of equal caloric content will result in identical weight change independent of macronutrient composition, and appeal is frequently made to the laws of thermodynamics. We have previously shown that thermodynamics does not support such a view and that diets of different macronutrient content may be expected to induce different changes in body mass.

Reduced thermodynamic efficiency will result in increased weight loss. The laws of thermodynamics are silent on the existence of variable thermodynamic efficiency in metabolic processes.

Conclusions

Dr.Jay Wortman another ex-Engineer.

In a former working life, Jay was an engineer. Dr Richard Bernstein was an engineer. Bob Krause was an engineer too. Is this just a coincidence? Perhaps not. This is purely anecdotal, but I have found that many of the most creative-thinkers in the area of health and nutrition have a background in engineering.

One of the sharpest minds in the blogosphere is Dr. Michael R. Eades. I’m sure you’re aware of his site, and would agree with him on most things regarding diet and health. He, too, began his career as an engineer.

Engineers must weigh different design choices on their merits and choose the solution that best matches the requirements. Their crucial and unique task is to identify, understand, and interpret the constraints on a design in order to produce a successful result. In other words, they make things work. Well the Doctors certainly make things work when it comes to the control of weight and diabetes.

Jay is not only a medical Doctor he is also a type two diabetic. He controls his diabetes with a lowcarb diet, exercise and no meds. Jay is fortunate to be on first name terms, with some of the people on the cutting edge of lowcarb research. Jay is far more knowledgeable than most Doctors regarding the lowcarb lifestyle, he knows it works, full stop. He is totally convinced it is the way to go, for diabetes and weight control. He also knows lowcarbing is 100% safe. How do I know that ? Because his wife and children are lowcarbers. Would a man with access to information many lay people never see, encourage his family to lowcarb, when he had the slightest doubt regarding long term safety, not in a million years.

Mandarin orange and cream lowcarb sponge cake.


Ingredients:
100 grams ground almonds
1 teaspoon baking powder
2 large eggs
1 tablespoon of melted butter
2 tablespoons of double cream
100 grams of tinned mandarin orange segments 

Method:
Mix all dry ingredients in a bowl.
Melt the butter, I use a Pyrex jug, add the eggs, cream, then add the dry ingredients and mix. Pour into a 6" microwave proof dish. Microwave in a 700watt for 3 minutes. Allow to cool and cut in half. Spread on extra thick cream and add 100 grams of mandarin orange segments, serves four. 
Another variation on our lowcarb almond flour cake recipes. 6-7 grams of carb  per portion.

"Abandon all hope, ye who enter Carbo’s blog"


Carbo aka Tubolard really scraped through the bottom of the barrel on his cesspit blog last night when he posted this from an anonymous person.

Some time after the above comment was posted, I received a telephone call asking had I seen the post. I had not, but going on to the cess pit, I immediately recognised the post was the handy work of our ex friend and ex team member Karen. The team distanced itself from Karen, some time ago, when some very worrying, and disturbing information came our way. Karen was not the hugs, kisses and cuddly person she appears to be on various forums and blogs.

I telephoned Karen and stated I believed she was the anonymous poster. She did not want to talk to me and hung up. After reflecting on the situation, I called her back and again a short call, but long enough to bait a trap. Would she be stupid enough to go back on Carbo’s cess pit and post a denial. She fell for the ploy hook line and sinker, and posted this.

The little, ugly twerp just phoned to give me a telling off or something, I told him I want nothing to do with him. He rang back, yelling that he is going to publiah my full name and address and my marriage certificate on his website tomorrow.
That will be very interesting, most likely my emp0loyees details whose address we have had packages sent to as we are out most of the day or out of the country.
Poor Eddie and Co, this has truly rattled his cage, only 3 people apparently knew about John'a suspected cancer then did they? Yeah right, Eddie the doughboy haa forgotten how many emails he sent out to multiple recipients then.

Now, there is no love lost between me and Carbo, but I have never considered him less than bright. Surely, if Carbo thought he had a potential libel case on his hands, why did he allow the posts to stand ? Why did he allow the posts to pass moderation ? Why did he not delete the possibly libellous posts and save himself possible trouble ? Why are the posts still on the blog around twenty-fours later ? Could it be Carbo is as bright and wily as me ? Could it be, Carbo, like any other straight thinking person, realised the first post was from Karen, and left her hanging out to dry in the wind.

Cheers Carbo, I owe you a drink, lowcarb of course.

A year in the life of a type two diabetic.

The diagnosis.

Hi Doc how did my blood tests turn out ? sorry Fred it’s as I suspected, I’m afraid you’re a type two diabetic. Um, so what do I have to do ? Take these Metformin tablets twice a day and come back in six months time. Is that it, is that all I have to do ? Well, cutting back on sugar will help you, get a bit more exercise in and change your diet, here’s a book sponsored by Takeda, it tells you what to eat. Thanks Doc, it says here to base my meals on lot’s of starchy carbohydrate foods and to eat spuds, bread, rice and couscous, that’s great Doc, there my favourite foods. ( Doctor looks slightly embarrassed and fakes a friendly smile)

So Doc, what’s all this malarkey about high blood glucose numbers ? It says here it can lead to blindness, kidney failure, heart attack and leg amputations, this is a wind up surely ! No, I’m afraid it’s true Fred. Jesus Christ !!! these Metformin pills must be amazing if they can stop all that. (Doctor struggles not to grimace) Tell me Doc, what causes blood sugar to go to dangerous levels ? Err, Err, Err eating starchy carbohydrate foods like spuds, bread, rice and couscous. Come on Doc your pulling my chain, the book says I should eat that sort of grub. See you in six months Fred, keep taking the pills. Gloria, send in the next patient please.

Six months later.

Good morning Fred take a seat, how are you doing ? Well to be honest Doc I am a bit worried, I still feel ill, my eyesight is still a bit blurred and I’m knackered out most of the time, even my old todger ain’t working so good these days. Well that’s par for the course Fred, diabetes is not much fun is it. Tell me Doc, I have been hearing about BG meters and test strips and that they can tell me a what my BG levels are, any chance of one on my prescription ? No Fred, for type two’s they are complete waste of time. Why is that Doc ? Well if you tested your BGs, and realised how high they were running, you would become very worried, you might even get severely depressed, and then I would have to treat your mental condition. Take it from me Fred, severe depression, is the last thing you want.

Doc. I joined a diabetes blog and website, and a geezer called Eddie , reckond cutting back on the grub, you and the dietition told me to eat, would help me no end, what do you think ? Um, I have heard of this Eddie, he his a total menace to the diabetes community, he is costing commercially lead forums and pharma companies a small fortune, he has no respect for most forum moderators and he’s a complete nutter, keep well away from him Fred, he’s trouble.

So this low carb lark he pushes is a no,no. Fred, what can some jumped up nobody, know about controlling type two diabetes ? Well Doc, he is always going on about a bloke called Bernstein and he reckons Bernstein is the man when it comes to controlling blood sugar levels, have you heard of Bernstein Doc ? Bernstein……...... Bernstein……..... didn’t he write West Side Story ?

Six months later.

Hi Doc, how was my latest HbA1c test ? Unbelievable ! looking at your tests, unbelievable, sorry Fred they must have got it wrong, these tests can’t be right. What's the problem Doc ? Well it says here your HbA1c has gone from 12 to 5.5, your trig levels have gone from 4 to 0.8, your HDL cholesterol has gone up and your LDL has gone down, these blood numbers can’t be right, it’s totally impossible to achieve these numbers. The last time I saw those sort of numbers they were for a 25 year old iron man competitor. Sorry Fred we will have to do the tests again.

Why are they impossible Doc ? I have been low carbing for six months. Lots of diabetics have done this, by low carbing they have lost weight, and years on, they are still doing well. Fred, do you seriously think, a bunch of low carb barmy army recruits, know more about diabetes control than I do ? Why do you think most of those low carbers have been banned from diabetes forums, I’m telling you, Fred they are nutters and trouble makers, have you seen the amount of fat those people eat ? Good grief Fred, those low carbers are dead men walking. That’s another thing Doc, they are always going on about walking. Exactly Fred, they have lost the plot !

Doc, I’ve been hearing about a guy called Gary Taubes, and he reckons it’s the carbs and sugar that leads to obesity, heart disease and type two diabetes, and not saturated fats. Ah yes, I have heard of Gary Taubes…………………… wasn’t he the drummer with Mott The Hoople ?

So, you don’t rate low carbing Doc ? Well if it works for you Fred stick with it, but no, I wouldn’t recommend it, who but a complete idiot, would take diabetes diet advice from some nutter on the internet ? Honestly Fred I sometimes wonder why I bother to turn up here some days. It’s patients like you that make this job a misery at times. On your way out, ask my secretary to come in would you, see you in six months Fred.

Gloria, place a note on Fred’s medical file. Patient far too happy and relaxed for a diabetic, suspect possible alcohol or narcotics abuse.

The King, no pun intended. Mr.B.B.King. I had the good fortune to see this stella guitarist and singer perform twice. Once with his own band at the Hammersmith Odeon and once at the Royal Festival Hall with his band, The Crusaders, and The Royal Philharmonic Orchestra during the recording of the album, Royal Jam. I will never forget those nights. BB started out picking cotton for a dollar a day, and become an international super star. He is still working at around eighty years of age. He is a type two diabetic.

We talk a great deal about the lowcarb lifestyle on this blog, and the need for regular exercise. With this in mind, we are delighted to welcome a new member to our team. The Internationally famous, Australian keep fit guru, Arthur Dunger. One point worth noting, is the fact that before lowcarbing, Arthur’s assistant Delilah was over 350 lb. National Diabetes Audit (NDA)

The NDA currently covers four ‘core care’ components of the National Service Framework (NSF) for Diabetes

1. Registrations:
Is everyone with diabetes diagnosed and recorded on a practice diabetes register?

2. Care Processes:
What percentage of people registered with diabetes received the nine key processes of diabetes care (Measure: Weight, Blood Pressure, HbA1c, Urine Albumin Creatinine Ratio (UACR), Serum Creatinine, Serum Cholesterol; Assess: Eyes, Feet, Smoking)?

3. Treatment Targets:
What percentage of people registered with diabetes achieved NICE defined treatment targets for glucose control, blood pressure and blood cholesterol?

4. Complications:
For people with registered diabetes what are the rates of acute and long term complications (disease outcomes)

The audits make grim reading. In all four ‘core care’ components there are serious problems.

1. Registrations:
Large % of diabetics are not diagnosed and recorded on a practice diabetes register.

2. Care Processes:
Large % of registered diabetics do not receive the nine key processes of diabetes care.

3. Treatment Targets:
Large % of registered diabetics do not achieve the NICE defined treatment targets.

4. Complications
Large % of registered diabetics have acute and long term complications.

To avoid getting overwhelmed with the wealth of data we will consider only the NICE clinical guidelines that the HbA1c level should be less than 7.5% (in itself considered by many to be too high!)

Over the years 2004 -2010 the % of registered diabetics failing to achieve HbA1c of less than 7.5% has remained at approximately 40% (with small year to year variations).

There seems to be only one conclusion – The NHS strategy for diabetes is not working. Given the increasing prevalence of diabetes the implications for both for the diabetics suffering and the public purse are deeply worrying.
